FLORIDA
CERTIFICATE OF IMMUNIZATION |

| |
Blue HRS Form 680
available from the Broward County Health Department or any
Florida doctor. Parent must provide proof of the following
immunizations: |

A. |
Diptheria, Tetanus and Pertussin
(5 doses required) |
| |
B. |
Polio (4 doses required) |
| |
C. |
Measles, Mumps and Rubella (2
doses required) |
|
*In addition children entering or attending 7th and 8th
grade will be required to have completed: |
|

D. |
Hepatitis B Series (3 doses that
take 6 months to complete) |
| |
E. |
Second dose of Measles vaccine
(preferably MMR) |
| |
F. |
Tetanus-Diptheria Booster |

| |
|
|
| 3. |
PHYSICAL EXAMINATION |
|
| |
If a student is entering a
Florida school for the first time he/she must present the
following documents: |
|

Proof that a school-entry medical
examination was performed within one year prior to the day
of registration. Physical must be done by a Doctor in the
United States or on a US military base overseas. |
|

|
|
| 5. |
SCHOOL RECORDS |
|
| |
Achievement test scores from the
previous school year, last report card, grades at time of
withdrawal. Students enrolling in exceptional student
education (ESE) classes, including gifted, must present
copies of the most recent Individual Educational Plan (IEP),
and Psychological Report. |

|
| |
|
|
| 6. |
TRANSFER |
|
| |
This is not necessary to enroll
at the beginning of the school year if you have not entered
another school. |
